This Privacy Policy applies to the services offered by CareLynx Pty Ltd ACN 136 444 543 (“CareLynx”).

As an Australian business, CareLynx is required to comply with the Privacy Act 1988 (Cth) (“the Act”) and the Australian Privacy Principles. CareLynx takes its responsibility for handling personal information seriously. This Privacy Policy explains how CareLynx may collect, use, disclose, store and otherwise handle personal information, including how you may access such information held by CareLynx.

Personal information is collected by CareLynx where necessary or related to one or more of its functions or activities. The types of personal information collected may depend on the type of services provided by CareLynx. CareLynx’s services include:
provision of the CareLynx Software Platform – web based platform and application;
training and support related to the provision of CareLynx software;
provision of consulting services;
provision of online support services; and
provision of support for self-assessment audits.
In providing the services, CareLynx may collect and be privy to personal information such as:
identifying information such as names, gender and dates of birth;
contact details such as phone numbers, addresses and email addresses;
employment and employee details;
government-issued identification (or identifiers contained in such) including drivers licences, Medicare cards, tax file numbers and health concession cards;
health service provider details such as membership numbers and AHPRA numbers;
sensitive information such as health conditions and medical records;
financial information such as billing information, bank account and credit card numbers;
transactional information; and
information about your usage of the CareLynx website, such as internet protocol (IP) addresses, web analytics data and any information you provide to CareLynx directly or indirectly through your use of the CareLynx website.

CareLynx recognises that individuals have a right to control how their personal information is collected and used. Unless given consent to do otherwise, CareLynx will only collect and use personal information as set out below in this Privacy Policy.

when using any service offered by CareLynx;

Personal information may be requested for a variety of reasons, including but not limited to; enabling CareLynx to provide a service or to update information presently held on file. You are not obliged to provide the requested information, however failure to do so may result in CareLynx being unable to provide services to you.
CareLynx may also collect information from third parties and public sources.
From time to time, CareLynx may collect unsolicited personal information. Unsolicited personal information is personal information received by CareLynx without CareLynx requesting it. If CareLynx receives unsolicited personal information, CareLynx will decide within a reasonable amount of time as to whether the personal information is necessary for the provision of the services or related functions. If CareLynx believes the personal information is reasonably necessary, it will notify you of the collection and retention of the personal information and will be bound by the terms of this Privacy Policy.

to promote and market all current and future CareLynx businesses, venues, products and services and to inform about the products and services of any of CareLynx’s commercial partners;

to enable CareLynx to comply with its obligations under law; and

CareLynx may employ or contract other companies or service providers to assist in providing CareLynx’s services, including but not limited to; marketing, market research, mail-house services, hosting and product development services, analysis of client lists and/ or consulting services. These third parties may have access to personal information that is needed to perform their specific function(s) and cannot use that information for other purposes beyond those functions.
The third parties to which CareLynx may disclose personal information to includes:
related entities of CareLynx;
employees, contractors and agents CareLynx engages to provide products and/or perform services;
professional advisors such as legal professionals and accountants;

government agencies, if required by law; and
any other person for any other purposes that would be reasonably expected (including if required by law).
CareLynx may disclose limited personal information including web analytics data to services providers engaged by or acting on behalf of CareLynx to assist it in developing and implementing software and updates.
CareLynx does not sell personal information to third parties.
Storage and Security of Personal Information
CareLynx takes all reasonable steps to keep secure all personal information and prevent the unauthorised access, modification, disclosure, interference, misuse and loss of any personal information held by CareLynx.
CareLynx has a variety of strategies and policies in place to protect the personal information it holds, including:
secure storage based in Australia via Google Cloud;
secure storage in locked areas for any hardcopy personal information;
system security with privileged and role specific access to personal information;
encryption for any personal information being transferred;
employee training on privacy and security of information, including confidentiality;
de-identification procedures;
contractual safeguards with the organisations to whom CareLynx are required to disclose the personal information for a permitted purpose; and
documented policies and procedures for information security.
CareLynx will only permit authorised personnel to access your information and information will only be disclosed to third parties where they have the appropriate authority.
CareLynx destroys or de-identifies personal information no longer needed by CareLynx, wherever possible.
Individual's Right of Access
CareLynx endeavours to ensure that personal information collected and used is current, accurate and complete. You and other individuals have the right to review, amend or delete personal information that may be recorded on CareLynx’s database.
Information may be reviewed, amended or deleted by contacting CareLynx via email at CareLynx.
A request may be made to delete personal, and all reasonable steps to delete the information will be made, except where the information is required for legal reasons. Deletion of information may result in CareLynx being unable to sell products directly and/or provide certain services including after sales services.
